Thank you for participating in our 2024 Pooch play offs to support the Animal Shelter Foundation. You will be invoiced for the contest fee upon the booking of your appointment with the photographer. The fee of $99.00 will go directly to the Animal Shelter Foundation. You may purchase prints and products of your dogs portrait session, however purchase is not required for participation in the contest, only the contest fee is required. 

Upon receiving this form the photographer will contact you by the phone number you provided. Please note the photographer will be calling with a 417 number. The photographer will have a short phone call to answer any questions, or address any concerns you may have and go over pricing and product options.
